By completing the fillble application below I understand that my application is complete only after I have provided all requested documentation and evidence of physical examination. This application will not be processed by the borough of Surf City or the fire company until the background Investigation is completed by the Surf City Police Department. The information I have provided on this application is true and correct, if accepted I agree to abide by the rules of the organization, the by-laws, standard operating guidelines, and instructions from the company officers or senior members as appropriate.

Background Check and Fingerprints

All applicants must submit to a mandatory background check with the Surf City Police Department for fingerprinting and background investigation. Your signature on this application authorizes the Surf City Police Department to conduct a background check to aid in the determination of your eligibility for a position in the Surf City Volunteer Fire Company No. 1 and Emergency Medical Services. The Police Department will contact you regarding fingerprinting and any necessary follow-up.

IMPORTANT Next Steps

• The borough of Surf City requires a paper application that has a notarized signature. 
• All applicants except Associate members need to have the physical test record signed by their physician. 
• If you are under 18, there is a form for your parent(s) or guardian to sign.
• Once you complete this document, it will be emailed to you. You will need to print it off and sign it along with getting it notorized.
